Care Notes for [Relative's Name]: May 2025 Summary

Relative's condition is worsening, especially stability and anxiety.

Current Needs & Observations:

1. MOBILITY/PHYSICAL: Needs full hoisting and two people to transfer to or from the chair. Even with specialist aids, communication is severely impaired; relies entirely on yes/no responses and simple hand gestures. The Physio score of 4 doesn't fully capture how bad it is day-to-day. We need the more complex wheelchair to make any difference.

2. PSYCHOLOGICAL/EMOTIONAL: Relative is experiencing high distress and confusion. They become acutely anxious when left alone for more than 45 minutes. Requires checking at least hourly, sometimes more frequently, especially overnight, for both safety and emotional reassurance. This level of **constant supervision** is unsustainable for me.

3. CONTEXT: Social services assessed last year but did not offer long-term funding for care hours, just respite. We are NOT receiving long-term care funded by local authority Social Services. I am funding all care needs privately and urgently need assistance for this high level of need.
