n the past, depression was often described as “melancholia”, which referred to severe sadness, low mood and loss of interest. Treatment historically included institutional care and “moral treatment”. During the 20th century, treatments such as antidepressant medication and ECT became increasingly common (Berrios, 1988).

Generalised Anxiety Disorder (GAD) was not always recognised as a separate diagnosis. In the past, conditions involving long-term anxiety and nervousness were described using terms such as “neurasthenia” and “anxiety neurosis”. The modern concept of GAD developed through changes in psychiatric classification during the 20th century (Rynn & Brawman-Mintzer, 2004).

The ICD-11 provides standardised criteria for diagnosing depression and GAD. However, one difficulty is that symptoms can overlap between disorders, and people with the same diagnosis can have different symptoms and levels of severity. This can make diagnosis and treatment decisions more complicated (WHO, 2024).

Clinicians manage these difficulties by looking at the whole person rather than only the diagnosis. They consider symptoms, severity, functioning, physical health, other mental health conditions, social circumstances and previous treatment. NICE recommends personalised treatment for depression, which can include psychological therapies and medication (NICE, 2022). For GAD, treatment can include self-help, psychological therapies such as CBT and medication depending on the person's needs (NICE, 2011).

by Affoue Denise Kouadio.

In the past, depression was often called "melancholia", while anxiety was describe as neurosis" or nervous disorders. Treatments included rest, structured routines and later medication (Royal College of Psychiatrists, 2023).
Today,, ICD-11 provides clearer diagnostics criteria, but diagnosis can still be difficult because symptoms vary between people and depression and anxiety can occur together (World Health Organisation, 2024}.
Clinicians therefore assess the whole person and may use therapy, medication and social support depending on individual needs (NICE, 2022}

by Shantell Madzwimbo.

How were depression and GAD described in the past?
in the past depression was often known as melancholia meaning a state if sadness and withdrawal of low activity. ( Royal college of Psycharasist, 2026)

How are they treated previously? In the past they was treated with just rest, isolation, sedation and institutional care but earlier was just introduced to anti depressants. . ( Royal college of Psycharasist, 2026)

what are the main problems with the ICD - 11 diagnosis’s? The ICD-11 provides a standard criteria which helps clinicians diagnose mental disorders consistently however depression and GAD can be overlapping symptoms which can also vary differently between each individual. (WHO, 2024;NICE, 2020)

How are they treated now in the UK?
in the UK they are being treated based on the NICE guidance with depression such as talking to therapist, CBT, antidepressants or a combination depending on how severe the depression is. (NICE, 2022;NICE 2020)

the overall treatments has changed from institutional approaches towards evidence based on psychology
